LOST COVID 19 VACCINATION CARD?

With all of the discussion ongoing about the possibility of booster doses of vaccines beginning next week, begin thinking that clinics may require you have you vaccine card on hand to be able to show immediate proof of your first 2 doses.

Booster clinics run by Marshall County Health Department will REQUIRE you to bring your card.  If you don't have your vaccine card, you will not be able to get a booster dose.

What happens if I have lost my COVID Vaccination card?

  1. Contact your vaccine provider and request a replacement card.  
  2. If you are a West VIrginia Resident, go to the following website and submit to WVDHHR for a replacement card.

Remember that staff have to research your past doses and it can take up to a couple days to get a replacement card, depending on the volume of requests.

You don't want to wait until the last minute to request a replacement card.
